*Attention current Monarch patients who are part of the Blue Shield health plan: Blue Shield of California has indicated it's ending its relationship with Monarch HealthCare. If you choose to remain a Blue Shield HMO member, you will not be guaranteed access to your Monarch HealthCare doctor throughout all of 2012.

You can stay with Blue Shield, OR you can choose one of the health plans that still have a relationship with Monarch HealthCare and will let you stay with your Monarch HealthCare doctor. You also will be able to see the same specialists you see now and get services from the same hospitals, or even additional hospitals, based on the plan you select.

Inquire with your Human Resources staff about the options that will allow you to maintain your important relationship with your physician.
